Individual to Individual
Business to Business

Individual Trader to Company
Company to Individual Trader
Customer Deceased

Once you have downloaded and completed the respective form, kindly also attach all the required documents mentioned in the same forms and send them back to Melita’s Customer Care team on info@melitaltd.com.
